need advice

We have a Parkinson's patient we have been seeing for 3 cert periods. She was started on peg tube feedings toward the end of the first cert period and then had multiple complications and med changes. She was also started on an experimental medication Duopa that is administered via the peg per IV pump. The SN has been seeing her BID to change out the medication cartridges because the patient doesn't have the dexterity to do it and her family is not always available. I'm thinking we need to discharge because the daughter can do it, she's just not always available. What are your thoughts on discharge?
