Good morning! Anyone here still using the form CMS-486? And what it is for? And is it still required? Need feedback on this. Thank you.

    It has not been required for years.

    It used to be a billing form used to summarize the care provided during the 60 days of the cert period and to justify why you were billing for services.

  • Ok. But what do you use if the patient have a change of condition and needs to update the plan of care? Is the PVO suffice for that situation?

    You should perform a SCIC and a physician verbal order will suffice to update the patient's plan of care.
